Michael Ombrellino, M.D., F.A.C.S.

Michael Ombrellino, MD, FACS joined the practice 2013 when the Vein Institute of New Jersey joined TCVCG. Dr. Ombrellino concentrates his practice on the advanced treatment of venous, arterial disease and aortic disease. 

He is a former Chief of Vascular Surgery at Morristown Medical Center, and remains an active member of the department as chairman of the Performance Improvement Committee and as a member of the Surgical Residency Education Committee.

Dr. Ombrellino was named “Teacher of the Year” at the State University of New York in 1998 and received the honor twice again as an Attending Surgeon at Morristown Medical Center, latest being in 2011. He continues to lecture nationally to both General/Vascular Surgeons and Surgeons in-training.

Dr. Ombrellino was born in Italy and grew up in Brooklyn New York where he continues to have ties. He is married, and both he and his wife enjoy travelling and gourmet cooking in their spare time. He is fluent in Italian. 

Clifford M. Sales, M.D., F.A.C.S

Clifford M. Sales, M.D., F.A.C.S., MBA joined the practice in 1993. Dr. Sales is active locally and nationally in the field of vascular surgery. His clinical expertise is in the management of lower extremity vascular disease and the management of vein disorders. 

He is actively involved in clinical research serving on many committees at the local, regional and national level. He has been voted by his peers a number of times for the Top Doctor award featured in New York Magazine and New Jersey magazine.

Dr. Sales is married with two children. He enjoys family time spent traveling, cruising and skiing. He spends much of his free time woodworking and enjoying many of the cultural highlights of New York City. 

Michael A. Pontoriero, M.D., F.A.C.S.

Michael A. Pontoriero, M.D., F.A.C.S. joined the practice in 1993. Dr. Pontoriero is board certified in both Cardiovascular and General Surgery. His area of expertise includes thoracic, aortic and carotid surgery, and lower extremity arterial reconstructions. 

Dr. Pontoriero is also an accomplished dialysis access surgeon. For several years, he has been recognized in NJ’s Top Doctor magazine by his medical colleagues. He is currently the director of the New Jersey Society of Thoracic Surgery.

Dr. Pontoriero has been Chief of Vascular and Thoracic Surgery at Clara Maass Medical Center (Barnabas Healthcare System) for many years, as well as a past President of the Medical Staff (2007-8).

Dr. Pontoriero is married with two daughters. He is a retired Major in the U.S. Army Reserves. Dr. Pontoriero is an active member of his Roman Catholic parish, and in his spare time enjoys running, reading, and traveling. 

Jonathan A. Levison, M.D., F.A.C.S.

Jonathan A. Levison, M.D., F.A.C.S. joined the practice in 1999. Dr. Levison is well versed in all techniques of endovascular surgery and holds an expertise in open aortic surgery.

Raymond J. Holmes, M.D., F.A.C.S.

Raymond J. Holmes, M.D., F.A.C.S. joined the practice in 2004. Dr. Holmes’ surgical interests include hemodialysis access, endovascular abdominal aortic aneurysm repair, and percutaneous peripheral vascular interventions.

Dr. Holmes is married with three children. In his spare time, he enjoys playing golf, kayaking, and surfing. He completed the New York City Marathon and is a competitive Triathlete. He was also finalist for the NASA Astronaut Candidate Training Program in 2004, and is certified in advanced open water diving.  

Michael D. Addis, M.D., F.A.C.S.


Michael D. Addis, M.D., F.A.C.S. joined the practice in 2005. Dr. Addis’ surgical interest is in endovascular treatment of abdominal and thoracic disease. He has presented this topic at various symposia and is serving as a sub-investigator in one of the industry-sponsored studies in which TCVCG participates. Dr Addis has also been voted by his peers as a Top Doc in New Jersey on several occasions over the past eight years.

Dr. Addis served as a Infantry Officer in the United Sates Army for 8 years after graduating from Dartmouth College. He graduated from the US Army Airborne and Ranger Schools before being assigned to the 4th Battalion 18th Infantry in Germany. He served during Operations Desert Shield and Desert Storm and was awarded the Bronze Star Medal for bravery during combat operations against the Iraqi Republican Guard in February of 1991. He left the Army to pursue a career in medicine.

Salvador A. Cuadra, M.D.,
F.A.C.S.

Salvador A. Cuadra, M.D. joined the practice in 2008. Dr. Cuadra’s surgical interests include minimally invasive treatments for peripheral vascular disease and abdominal aortic aneurysms. His expertise also lies in the management of patients with carotid artery disease and hemodialysis access.

Dr. Cuadra is married with two children. Dr. Cuadra is a wine connoisseur and enjoys the arts greatly. He is an avid golfer who rarely allows inclement weather to deter him from the links. 

Combiz Rezayat, M.D., F.A.C.S.

Combiz Rezayat, M.D., F.A.C.S. joined the practice in 2010. Dr. Rezayat has extensive experience with the management of complicated thoracic and abdominal aortic aneurysms as well as the endovascular management of lower extremity arterial occlusive disease. 

He has interests in the management of aortic aneurysm, mesenteric ischemia, peripheral artery disease, carotid disease, and pulmonary embolism. He has published and presented in many of these areas of interest.

Dr. Rezayat enjoys painting, traveling and sports. He is particularly fond of acrylics and is an accomplished artist . He is married and has a son. 

Scott A. Sundick, M.D., F.A.C.S.

Scott A. Sundick, M.D. joined the practice in 2010. He is highly trained in endovascular repairs of aortic and thoracic aneurysms as well as carotid artery disease. In addition to lower extremity angioplasty and stenting, Dr. Sundick expertise lies in minimally invasive treatment of aneurysms of the spleen, kidneys and liver, and the treatment of liver cancer and upper gastrointestinal disorders.

Dr. Sundick is among the first generation of vascular surgeons who consider themselves “interventionalists” in addition to surgeons. This approach to patient care gives him many tools to diagnose and treat vascular disease, but offers the patients the best of both worlds.

Bruce Brener, MD, FACS

Bruce Brener, MD, FACS joined the Cardiovascular Care Group in January 2016. He is an experienced, well known vascular surgeon who continues to advance traditional and minimally invasive surgical techniques for the treatment of vascular disease. He is firmly committed to providing top-notch, state of the art comprehensive, compassionate care to his patients.

Cindy Sturt, MD, FACS

Cindy Sturt, MD, FACS joined the Cardiovascular Care Group in January 2016. Dr. Sturt is a comprehensive vascular surgeon with both open and endovascular skills. She treats the full spectrum of vascular pathologies from venous disease, dialysis access, peripheral vascular disease and aortic disease, varicose veins, sclerotherapy for spider veins, utilizing the most recent and effective vascular technologies.

Daniel I. Fremed, MD, FACS
Daniel I. Fremed, MD joined the Cardiovascular Care Group in 2017, after successfully completing his residency in Vascular Surgery at Mount Sinai Hospital, New York, New York. He is board-certified in vascular surgery. His clinical interests include abdominal and thoracic aortic aneurysms, peripheral arterial disease, carotid artery disease, mesenteric and renal artery disease, dialysis access creation and maintenance, as well as disorders of the venous system. He has published and presented in many of these areas of interest.

Dr. Fremed returns back to his home state of NJ to practice vascular surgery. Dr. Fremed is an avid traveler and enjoys live music. He loves the great outdoors and participating in competitive ice hockey.

Neil H. Poulsen, MD

Neil H. Poulsen, MD joined the Cardiovascular Care Group in 2021, after successfully completing his vascular surgery fellowship at Carolinas Medical Center and general surgery residency at St. Barnabas Medical Center. 
His clinical interests include thoracic and abdominal aortic disease, carotid disease, peripheral vascular disease, thoracic outlet syndrome, dialysis access, and deep venous disease. He is highly trained in both endovascular intervention and open vascular surgery techniques.

Kunal Vani, DO

Kunal Vani, DO joined the Cardiovascular Care Group in 2021, after successfully completing his fellowship in vascular surgery at Pennsylvania Hospital and residency in general surgery at Rowan University.
His clinical expertise includes management of aortic and visceral artery pathology, lower extremity peripheral arterial disease, renal/mesenteric disease, and dialysis access. He is highly trained in both endovascular intervention and open vascular surgery techniques.
