If you had the spider veins treated with just laser without addressing any underlying "feeder" reticular veins it is possible to have veins reopen. The standard of care for spider veins is sclerotherapy to reticular and larger spider veins with laser as a nice "clean" up of finer veins. It typically can take 3-5 treatments on average. Additionally not all lasers are able to treat effectively. It is better to be treated in a clinic where the practitioners are trained to do all modalities of spider vein treatments and that there is a surgeon available to ultrasound and look for perforator and communicating veins in areas that are not responsive to treatment.

The gold standard treatment for spider veins is sclerotherapy. Topical laser is good for only the very small veins and not the larger spider veins. Endovenous laser treats refluxing valves and not spider veins. Regardless of the treatment there is always the chance of some spider vein recurrence and spider veins usually require maintenance touch up treatments.

Yes. I believe laser ablation of spider veins on the legs is relatively ineffective. I use injection sclerotherapy. Be advised that people with spider veins tend to develop new spider veins over time, if it is genetic.

Yes, spider veins can return after treatment, whether by laser or sclerotherapy. Sometimes they do not all close completely and /or you can form new vein with time. Compression hoses are very important part of spider vein treatment to ensure that they close and to minimize the chance of recurrence.
