Which modality and in given modality, what frequency is superior for end result to a given patient in EVLT? Does the ownership of a particular machine influence opinion?

There are many ways to close a vein—EVLT, RF, Clarivein, glue, and Varithena. They all work, and their use depends on the experience of the physician using them. I prefer laser as my number one choice and Clarivein as my second. I use a wavelength of 1470 which I think is the ideal wavelength to treat veins, but there are other laser wavelengths which can also be used.

Wavelength is all that matters with laser. I think it's pretty much established that the 1470 nm wavelength which targets water and uses less energy is the best way to go.

Of course those who have a particular machine that is what will be recommended. It is not so much the laser used, but the experience of the surgeon. 1470 nm is pretty much standard with laser, RF is also popular. Again, a surgeon experienced in laser physics, appropriate tumescent anesthetic, and ultrasound certified (RVT) will produce excellent results and low complication risk.

It's always nice to have a practice that use all 3 different technologies: EVLT which uses laser energy, VNUS Closure which uses radiofrequency energy, and ClariVein which uses mechanochemical energy. Any one type of energy source can be used to achieve best results for a particular patient.

Research I presented last year at the ACP conference showed that ELVeS which is the 1470 nm LASER had the least discomfort associated with the closure procedure. patients rated 0.5 on a scale of 1 to 10 which compares favorably to the EVLT 810 nm diode LASER which was rated on average at 2 on a scale on 1 to 10. I have both machines but now almost always use the ELVeS but obviously I can't use a machine i dont own.

For the various lasers, higher wavelength tends to cause less pain/bruising following the procedure. This is similar with the VNUS ClosureFAST. So as long as you are gettting treated with the most current generation laser or RFA you will be very pleased. As for long term results, all modalities, including first generation lasers, have great results.
