Most insurances cover treatment. As for if you need a referral, that depends on your insurance. Some require a referral others do not. I would recommend calling your insurance carrier.

If you belong to an HMO, you probably need a referral. Most other insurance agencies do not require a referral. You can just make an appointment.

I can't answer that question because it solely depends on your insurance. Managed care is typically run through a PCP. HMOs are examples. Other insurers, including Medicare, do not typically require a referral. That question needs to be directed to your insurance carrier.
