Depending on your insurance coverage, you may need a referral to see a vein specialist for evaluation as you would for any specialist. If you see a vein specialist, most practices will handle the workup and obtaining pre-authorization for treatments

Most insurances cover treatment. As for if you need a referral, that depends on your insurance. Some require a referral others do not. I would recommend calling your insurance carrier.

I can't answer that question because it solely depends on your insurance. Managed care is typically run through a PCP. HMOs are examples. Other insurers, including Medicare, do not typically require a referral. That question needs to be directed to your insurance carrier.
