Double Saphenous Vein and EVLT.

I had EVLT done in March. I have a double great saphenous vein. They only did one of them. Now my legs feel (and look) worse than they did before the EVLT. I'm switching docs, but what are the chances of EVLT working on the other vein in that leg?

Depends on amount of reflux but should help them if it is significant.

Double great saphenous veins are not uncommon. Often times both veins need to be treated with either EVLT or radiofrequency. If only one is treated it is not unusual the leg will feel or look the same as if nothing would have done.

If you still have venous reflux into the second saphenous vein, then your best choice will be to have it treated as well.

It sounds like you need the duplicated vein ablated.

If the great saphenous vein which remains is shown to be incompetent on ultrasound then sealing it should improve your symptoms and appearance. A second opinion with an experienced surgeon is strongly recommended.

Good if that's the problem. You need another ultrasound to check both veins again, and to guide future treatments.

The need for another intervention depends on your initial reflux study and whether the said duplicate vein demonstrated reflux. the other interesting aspect to look at is the state of the small saphenous and the accessory saphenous vein if you have one along with any perforators.

I have seen a duplication of the saphenous vein several times and some say that the incidence can be as high as 10%. If the duplicated saphenous vein has reflux as demonstrated by venous duplex then it too should be treated by EVLT. I am not sure how long it has been since your EVLT, but the leg usually looks worse for several weeks before it looks better.
