How strongly would you recommend this procedure pre-pregnancy?

During my last pregnancy I had severe varicose veins & got a few superficial blood clots in my right leg. I had the veins removed. Now, I am starting get another large but painless varicosity in my left leg. Should it be removed before another pregnancy?

Your should be seen by a vascular trained surgeon who specializes in treating varicose veins. We see many pregnant women with varicose veins. They may resolve after the first or second pregnancy but then remain. With your history of clotting, I recommend several things. Have your doctor test your blood for clotting factors. Get an ultrasound to rule out underlying vein reflux. If present, this is sealed using laser and the bulging veins are removed. This is done in the office with local anesthesia, not in a surgical center. You should also habittually wear good quality compression stockings. We use thigh length as it is better tolerated. At least 20-30 mmHg, but possibly moving to 30-40 mmHg as your pregnancy progresses. This is an inherited condition. Ask your parents.

If you have varicose veins then you should see a vein specialist for a full venous evaluation, including a venous ultrasound. If you have reflux (saphenous valve malfunction) and varicose veins, then these should be treated before your next pregnancy.

I would certainly urge you to have this evaluated and treated before another pregnancy. Pregnancies tend to enlarge the varicosities and predispose you to thrombophlebitis (blood clots).
