I have small pieces of tissue coming from the microphlebectomy incision sites, should I be trimming it? What to do about pain?

I have small pieces of tissue coming from several incision sites. I can pull them and trim some, but then they bleed a little. Should I be trimming this tissue? Also, what about the incision site on top of my foot that hurts a lot and sends shooting pain down my foot?

Your surgeon can trim them at your follow up appointment. Don't just pull them. They will bleed and hurt. They need to be trimmed which is painless. The foot incision is in an awkward spot so just avoid irritating it and keep the area clean and dry. Ibuprofen or Aleve can be used for pain as needed.

Do not pull these small "strings." Trim them with a pair of clean scissors, close to the skin. It is just a little tissue. The feet have a lot of nerve endings and less padding than higher in the leg. My patients massage with arnica or Dermaka cream several times per day and this desensitizes the little nerves in the area for faster healing.

I would strongly advised against doing that. Let your doctor do it. It is his responsibility to take care. You might end up infecting the incisions.

The skin that you see in the microphlebectomy sites is the shriveled remains of the avulsed vein. Your doctor should trim these. If you pull on them they may bleed or cause pain. If left alone, they will fall off on their own. Incisions on the foot are usually more tender and take longer to heal.
