I think the compression hose is causing more pain?

I had EVLT greater saphenous vein on right leg in mid May 2011 and two phlebectomys on the ankle. I have A LOT of spider veins on the anterior dorsum of the ankle and it still aches daily. Why does wearing 15-20mmHG hose make it hurt worse? Is this normal?

Treating the spider veins with sclerotherapy will be a more direct approach to treating this problem than wearing compression stockings.

The veins on the top of your foot should not hurt. Therefore, we would first want to be certain that the compression hose is fitted properly. Hose that is too tight will cause pain. Secondly, we would look for another source of your discomfort. There may be underlying soft tissue or bone pain that you are attributing to your veins.

I don't know why your leg hurts more with compression hose. It shouldn't and
possibly they don't fit properly. I have seen this in a few patients.
Sometimes changing to a different brand of stocking helps and rarely I have
seen patients that simply can't tolerate compression because of nerve
problems etc.

It may be possible that this isn’t related to your veins but changes in you ankle. Perhaps have this investigated. 15-20 compression is low and considered sub-clinical but it should still be measured and fitted correctly and be good quality fabric. Also have the venous circulation checked for perforator veins as these can contribute to spider veins and discomfort in the ankle.

It is not normal for a compression hose to cause discomfort especially a low compression as you describe. EVTL and microphlebectomies do not remove spider veins. You may require sclerotherapy for this. As far as the ankle discomfort, you should return to your treating vein specialist for reevaluation especially to determine if you have any nerve irritation.
