The laser is a light source and heat is generated from the laser. There are different kinds of lasers and different strengths. There may be some discomfort from a laser treatment, but either topical or tumescent anesthesia can be used to alleviate pain, and when treatment is done by an experienced physician, there should be very little pain.

Sclerotherapy with an experienced practitioner is minimally uncomfortable. It is still the gold standard for treating spider veins. Laser is a nice modality for very fine red/blue veins as long as there are no "feeder" veins underneath. These should be treated or the areas will continue to open up. I tend to use laser as a "clean-up" after a series of sclerotherapy. The right laser is important. I have a cooling tip on my Cutera 1064yg laser which makes it much more comfortable along with a layer of cold gel. It still will feel like a snap or a small rubber band. I also have a Dornier 940 diode which has a very small spot on it and it is wonderful for facial spider veins. Again, an experienced practitioner can make this quite comfortable. We avoid using topical anesthetics as there is some constriction of the vessels associated with them and they are minimally effective.

If you're asking about the Endovenous Laser, it's not very painful. More like mildly uncomfortable for most people.
If you're asking about the Cutaneous Laser, or an IPL they are a little more uncomfortable. It feels like grease popping on the skin. However, most people tolerate it well. If it bothers people you can use an anesthetic cream before the treatment which helps.
