Is treatment possible if I have venous reflux and chronic DVT?

I have been diagnosed with reflux; it was confirmed by in-office Doppler/Duplex ultrasound. The findings show chronic deep vein thrombosis in my right leg, common femoral and external iliac vein. I'm having significant leg pain at night. Is treatment possible given chronic clot status? Stent?

Treatment is possible, but that would be strictly up to your physician. Did you have a filter placed? Sometimes the clot can be broken up with a roto rooter-type tool and medications. Sometimes the superficial reflux can be treated even with DVT, but that is a decision done carefully by your treating physician, which is typically an interventional radiologist.

You have a significant problem, one that is not easy to resolve. If the only venous drainage of your leg is the superficial saphenous system, then this should not be treated for reflux. You need to be evaluated by a vascular surgeon or vein specialist to see if the deep system could be stented. Sometimes venous bypass is possible, but the long term results of this are not good. Usually, over time, new veins will form around the venous blockage to improve the venous return, but it will never improve to your pre-clot status.

It becomes more complicated to treat reflux in the presence of DVT. Treatment also depends on the etiology and degree of obstruction. You might need a venogram to better assess the anatomy. Additionally, you might need to wait 1-2 years until the DVT clears before treating the reflux.
