Post-Thrombotic Syndrome

Does EVLT for superficial phlebitis also help with post-thrombotic syndrome in a deep vein? Is ultrasound of deep and superficial systems routinely performed before deciding on EVLT? Thanks.

Ultrasound is the best way to evaluate both the deep and superficial (saphenous) systems. This may be done laying down and standing (for the superficial systems). If you do have post thrombotic syndrome from prior deep vein blood clots this is usually managed with compression hose (30-40 mmHg) to reduce swelling and chance of developing an ulcer (wound). If you have superficial reflux in the saphenous veins with associated varciose veins you may be a candidate for EVLT, but I recommend your evaluation and possible treatment be done by a board certified vascular trained surgeon.

It will not affect the deep veins if they have insufficiency,. It may help some symptoms, however if there are combined deep and surface valve problems.

Yes an ultrasound should be performed to check both the deep and superficial cystems of veins prior to any procedure. Especially since if the deep veins are blocked it may be contraindicated to treat the superficial veins, as these may be important collaterals for the venous circulation of the legs.

EVLT for superficial phlebitis will treat reflux of the superficial veins , usually the greater or smaller saphenous or branching refluxing veins. It will reduce the venous hypertension in the superficial system which can be contributing to the total venous hypertension. EVLT will not treat the deep system. We routinely evaluate both the deep and superficial in our venous examination.
