What is best treatment for lipodermatosclerosis?

For 4 yr I have had inflamed nodules turning into sclerotic tissue. Biopsy first interpreted as Erythema Nodosum, then another path calls it LDS. Full venous studies negative for CVI-perfect veins.Is it reasonable to continue taking Trental..or what?

Difficult question. LDS is usually associated with venous reflux. I have seen it in people who are very overweight as this can cause venous hypertension. Compression is still important and Trental may help. Topical steroid creams may help inflammation. Capsaicin may help pain and horse chestnut extract is shown to be of benefit in some patients.

Compression probably the best option. Might consider Lymphedema treatments as well.

I would not continue trental. It is difficult to say exactly what is your problem form your description. Lipodermatosclerosis can be due to chronic lymphedema and also to deep or perforator vein insufficiency You need to see a vein specialist. Inflamed nodules is not characteristic of lipodermatosclerosis.
