While painkillers have limited effects, hormonal therapy has been found to be somewhat effective in limited trials. See your gynecologist for further recommendations.

I would recommend you to an Interventional Radiologist who specializes in PCS for evaluation and treatment if appropriate. They will typically do testing and be able to treat at the same time if they find problems (stenting usually). Long standing use of pain medications can cause a lot of problems so why not try to deal with the cause of the problem?

You might benefit from a procedure that ablates the offending veins in your pelvis. You first need to be sure of the diagnosis. I definitely would not start any narcotics. You have to be sure you don't have some type of GYN issue first.Then ultrasound or possibly CT/MRI imaging to make the diagnosis.
