I have a client whose legs increase in size during the day a lot. By the time I see him (around mid0day) he has 3 times the swelling. He just started to wear TED-like hose, but previously wore support hose. What time of day should I measure him for knee-high stockings?

Measurements to fit compression stockings should be taken in the morning before swelling occurs. Stockings applied in the morning will prevent swelling and must fit properly before swelling develops.

Measure in the morning since the legs tend to swell during the day. If not fitted in the AM, the hose will be loose in the mornings when the patient puts them on (because the swelling has subsided). You might have to upsize if the patient can't tolerate a given size or compression. Ankle/brachial index is advisable to measure.

He should be measured first thing in the morning, preferably just after he gets up. Or, he can be fitted if he dons the TED hose before getting out of bed and gets measured as soon as possible in the morning before the swelling has time to increase. It may be that he will need to be re-measured more frequently as the swelling is controlled.

The ideal time to be measured is when the leg is at it's smallest. Compression stockings should be applied first thing in the morning after the legs are elevated at night; therefore, measure first thing in the morning before the legs swell.
