Why is it necessary to wear Compression Stockings after Sclerotherapy?
Compression stockings - at least theoretically - by compressing a vein, reduces the volume of blood that can remain in the vein after sclerotherapy. The less blood in the treated 'denatured' vein, the less blood that must be removed as they are removed together and the sooner that this removal can occur.

Compression sclerotherapy is thought to be helpful after sclerotherapy for several reasons. It helps in keeping the treated vessels collapsed and promotes their faster healing and disappearance. It is also thought to offer an additional protection against deep vein clotting ( besides ambulation and physical activity) ,as it prevents stagnation of blood in the legs. There has been at least one good study that showed better cosmetic results for patient who wore compression stockings after sclerotherapy.

The benefits of compression stockings after sclerotherapy are so numerous and well documented that it is now considered to be the current standard of care. Some of the benefits include: direct apposition of the treated vein walls to allow effective fibrosis and vein resolution, a lower incidence of post-sclerotherapy pigmentation, and a lower incidence of deep vein thrombosis by encouraging good blood flow through the deeper veins. Many Phlebologists recommend a 1-3 week period of compression after sclerotherapy.

Studies have suggested better cosmetic outcomes. The duration of wearing these stockings is controversial.

I recommend wearing compression stockings for 48 hours after sclerotherapy. Compression helps to keep the veins reduced in size and the sclerosing solution in contact with the injected veins so as to get a better result. Compression also helps to reduce the potential complications from sclerotherapy such as matting, staining and new vein formation(neovascularization).

For the very small spiders it probably is not necessary. Larger veins however may heal better if compressed after.

The object of sclerotherapy is to cause the cells lining the abnormal veins to stick together while the vein shrinks and scars itself closed. The external compression helps to empty blood from the veins and it compressses the treated veins so the vein walls are more likely to seal shut.

It supports the veins in a compressed or closed position to allow improved sealing of the veins after administration of the medication. Also it reduces the small risk of blood clots after the procedure.
