Wound care for skin ulcer caused after the sclerotherapy

An ulcer formed after sclerotheraphy and got infected. I took antibiotics and it still wasn't healing right. The Dr. had to get in there and got rid of the bad tissue, but it's still bleeding a day later. It's bleeding through the unna boot and wrap. Is this normal?

While rare, ulceration can happen after sclerotherapy. If the area is small just a wrap with silvadene cream 1% is enough. Washing thoroughly with soap and water several times a day. It will heal. The redness is typically not infection but inflammation in response to the ulcer. A larger area may need an UNNA boot. Either way it will take a long time to heal. A little firm pressure over the area bleeding will help. Go back to your surgeon for a dressing change and evaluation.

I would apply silvadene cream and wear compression hose or an unna boot
wrap. If you have not had an ultrasound of your leg to look for leaking
veins (venous insufficiency) you need one and probably a new vein doctor
too. Send me a photo of your ulcer via email and I should be able to give
you a better opinion.

An ulcer can form after sclerotherapy and may require excision. Unna boots work by absorbing the fluid from an open ulcer and allowing the zinc and calamine in the unna to soothe the ulcer. It is not uncommon for an unna boot to require weekly changes for 4 to 6 weeks and longer. Eventually as the ulcer closes, the drainage stops.
