If you have deep vein reflux, then you need all the veins you have to move that blood back to your heart. By removing your superficial and relying on your refluxing deep system to return the blood to the heart, it can pose a problem. Doctors do not want to take any pathways away if your deep system (the very important system) is refluxing. It would be contraindicated. But for some physicians if the deep vein reflux is minimal to the point that they think it would pose an issue, then they might go ahead with treating your superficial veins. Its at the discretion of the doctor to decide whether or not it would pose a future problem with your deep system.

If your deep veins are compromised, I would not recommend ablation of the saphenous veins. You would need to be evaluated by a board-certified surgeon, one who is vascular-trained. Evaluate risks versus benefits together. This is too difficult to answer in a blog. I do recommend you be in at least a 20-30 mmHg thigh-length compression hose. Get fitted by someone experienced so you get something that is comfortable and appropriate.

Yes, you can just have the smaller superficial veins treated. This is generally considered cosmetic by insurance companies in the U.S., but not the rest of the world.
