There are techniques to remove some thrombus at time of injection. Otherwise at about 10-14 days is when most patients notice darker, tender and palpable veins and this is a great time to drain them. Healing is faster and there is reduced chance of pigmentation of the skin (brown marks). These will often resolve with time.

Very few people that I treat ever develop a "thrombus" post sclerotherapy. If anything, they develop "trapped blood." Trapped blood is not a blood clot. It is tired blood that just happens to have been trapped within the resultant scar tissue that develops after sclerotherapy. This trapped blood should be removed for comfort and cosmetic reasons usually within a few weeks of onset.

The thrombus doesn't always need draining. I usually recommend draining if the site is larger than a thumbnail bed, painful, or discolored.

If your treated veins are bulging, hard and painful, you may want to consider an incision and drainage under local anesthesia. This provides almost immediate relief.

Post sclerotherapy evacuation (drainage) of thrombus should be done as soon as the thrombus is recognized. I usually drain these anywhere between two and six weeks after the sclerotherapy.
