It is essential that the pressure created by the compression stockings must exceed pressure of the treated veins in order for these vein to collapse. Your treating physician can determine so-called endovenous pressure precisely. Depending on the agent(s) being used for injections and subcategory of the treated spider veins, compression therapy ranges from 4 to 14 consecutive days.

In our practice, we use medical grade stockings with 20-30 mmHg pressure. Many clinics vary, but 15-20 mmHg pressure is the least that should be considered. Length of time varies in our practice to the type of veins treated. For very superficial spider veins without much reticular feeder veins, the stockings can be worn for 3 days. For larger vessels, the stockings can be worn for one week. During those periods, we tend to have our patients wear them 24 hrs per day and they don't even have to learn how to put them on.

There have not really been any trials done on the effectiveness of hose against no hose, but we do believe (and it is generally accepted in the vein community) that it is beneficial as they keep the smaller superficial vessels closed down while the solution works.

We feel you should be measured and fitted by an experienced fitter to ensure the correct size and strength. Poor fitting hose are uncomfortable and it discourages compliance.

Wearing graduated compression stockings is a good idea after sclerotherapy. You want to keep blood flow from going into the treated veins, so the sclerosing agent stays in place or inside the injected vessels without being washed out. External compression favors the collapse of the small vessels. Several studies have shown that moderate graduated stockings improve the final outcome after a sclerotherapy session when used for two to three weeks after.

Sclerotherapy involves injecting medication into smaller veins, displacing the blood. Keeping the veins collapsed as much as possible with firm compression 30 mmHg is beneficial. Wear the stockings for 2-4 weeks.

We recommend 20 - 30 mm Hg, which is considered moderate. However, some surgeons recommend 30- 40, which is firm. There is more evidence to support both compression pressures, but we find compliance/adherence is better with the 20 - 30mm Hg. The maximum benefit was seen by wearing the stockings for 3 weeks, although most of the benefit accrued by one week.

Moderate compression stockings are usually sufficient after sclreotherapy. I usually recommend my patients wear them for at least 2 weeks after any venous procedure.

Following sclerotherapy, I recommend wearing compression hose 20-30mmHg for 48 hours. I also recommend no soaking in a hot bath or sitting in a hot tub for 48 hours after treatment.
