Why am I getting blood clots in my calf WHILE on warfarin?

I have been on blood thinners for over 15 years for DVTs in my leg, most likely caused by my Factor V Leiden and incompetent veins, and wear my compression stocking everyday. In the last two years, I started getting superficial blood clots in my calf. I have a blood specialist, have seen a vein internist, have had lots of tests and no answers, just got another clot last week. What should I do?

Being Factor V positive, you are obviously at greater risk of blood clots. Thus the need for Warfarin. Assuming you are therapeutic on the Warfarin, it does not mean that you still cannot clot. The fact that you have vein issues only increases your risk of clotting off those larger affected veins since the blood is just sitting in those veins not moving. You may just need to have your veins addressed. Your vein internist may not know what to do.

An important piece of information would be how often your INR is checked and how often you are sub-therapeutic. If you are being checked 1/month, that's not enough. You would need to be checked at least every week for a few months. You may need an adjustment in the warfarin dose. There are also some acquired conditions where warfarin is not an effective blood thinner. You may need to be changed to another anticoagulant.

The fact that you have Leiden V makes you prone to clotting even while on anticoagulation. If you have varicose veins in your legs that are clotting to form superficial phlebitis, then you may need to consider treating these. See a vein specialist.

Are you therapeutic on the Warfarin? Have you had the incompetent veins treated? Are you avoiding sitting and or standing in one position for long periods of time? Are you getting regular exercise, especially walking? I need more information before attempting to answer your question.
