I have distended, ropey veins on the anterior surface of my left calf and have been told that my problem involves the perforator veins. How would you treat this? Would you use traditional laser or "cold" laser?
Perforators can be treated with endovenous laser, VNUS RF or sclerotherapy. I know of no role for cold laser. I would ensure first that there is no associated saphenous vein reflux.

The perforators would be treated in conjunction with the likely saphenous vein disease. Laser or sclerotherapy can eradicate perforator disease.

Options for closing a perforater vein include: laser, radiofrequency ablation and sclerotherapy. Also, when varicosities are removed by microphlebectomy, it is not unusual to also remove the peforator.

Some time leg veins are feed by perforators as there primary source of reflux. Depending on the size, location and configuration, I treat them with endovascular laser, rf or sclerotherapy.

Any of the medical LASERs used for endovenous thermal ablation procedures can be utilized well for ablation of perforating veins. Other options include radiofrequency ablation with the VNUS RFS device or foam sclerotherapy. Low level or "cold laser" are not used to treat veins.

Incompetent perforator veins can be treated various ways including laser ablation, radiofrequency ablation, as well as other surgical techniques.

These can be treated with the Vnus perforator device laser ablation or Sono guided foam injections.

Endovenous laser ablation is not cleared by the FDA for perforator ablation. The only device that is FDA approved for PERFS is the VNUS RFS device. We've done thousands of these at the various CVD centers. We train other doctors on how this device is used properly.
