Should I have another vein embolization done?

I had an ovarian vein embolization done 5 months ago and now the pain is back worse than before. Should I consider having another one done, or is a hysterectomy a better option?

If it was me, I would have another CT venogram to assess the situation.

You should discuss this with your vascular surgeon or interventional radiologist.

It really depends on what and where your pain is. People suffering from PCS often find that they also have varicose veins in and around their groin region. Many of these veins can often be treated, resulting in a decrease in discomfort especially during their period. Having an embolization procedure only treats one of many veins inside the pelvis that is causing your pain and may never even address those problematic veins actually generating the pain. Many of the women I treat for this issue find relief with just treating their vulvar veins because those are the veins that are actually causing their immediate pain. Consult with an experienced and accredited vein specialist to determine the appropriate course of treatment.

If you follow up with your physician they will probably have you get another ultrasound and possibly another venogram as well. A hysterectomy could be an option but does not always relieve all of the pain, especially if there are veins still present that have different sources. Another embolization would probably be the first route to take before a hysterectomy, unless recommended otherwise by your doctor.
