Why can't EVLT and sclerotherapy be performed on the same day?
I have reflux of the GSV. My doc recommended I be treated with EVLT just below the knee, then weeks later treat the remainder of the calf with foam sclerotherapy. This sounds reasonable, but why can't the procedures be performed on the same day?

My preference, based on guidelines, would lead me to recommend upper leg greater saphenous ablation first.

If your insurance is paying, each procedure has to be performed separately as the insurance will not pay .

It's reasonable. Some doctors do both on the same day. However, there have been some reports of increased risk of DVT when done at the same time.

Often when you take care of the area where the problem originates, it may diminish or eliminate any reflux further down the leg. It is not a good idea to do EVLT below the knee in the front, as there are superficial sensory nerves close by the vein that could be affected. A repeat ultrasound following the EVLT would determine if further treatment is necessary.

I routinely ablate a great saphenous vein to just below the knee and then I foam the vein from this point distally. I do both at the same time. There is no reason to do the procedures separately if the original ultrasound demonstrates reflux below the knee.

I had EVLT about 2-3 weeks ago and I'm still experiencing sharp pain. It's mostly when I stand or walk. Even when I wear a compression stocking during standing or walking, there is no relief.
